Insurance
I am an In-Network Provider for Premera. For those using Premera, I will submit paperwork on your behalf. Please note that missed or late cancelled appointments will be charged at my full rate and are not covered by insurance.
For out of network insurances - Submitting for insurance reimbursement will be your responsibility. I will provide you with a receipt of all services and information needed for your reimbursement.
Questions to ask your insurance provider:
Do I have mental health insurance benefits? What are my out of network benefits? How much will be covered per session?
What is my deductible and has it been met?
How many sessions per year does my health insurance cover?
Is approval required from my primary care physician?
Your insurance provider may also want to know the following details:
My name and credentials: Shannon Shively, M.A.Ed., LMHC
My license number is LH60668882
My NPI number is 1811388689
Most common procedure codes will be either 90834 or 90837
Cancellation Policies
The appointments you book are held for you, and can rarely be filled with short notice. For this reason, I hold the following cancellation schedule:
No shows - full session fee
Within 48 hours - $100
Within 7 days - $50
Please note that there are no exceptions to this policy.

STANDARD NOTICE
“Right to Receive a Good Faith Estimate of Expected Charges” Under the No Surprises Act
You have the right to receive a “Good Faith Estimate” explaining how much your medical care will cost
Under the law, health care providers need to give patients who don’t have insurance or who are not using insurance an estimate of the bill for medical items and services.
You have the right to receive a Good Faith Estimate for the total expected cost of any non-emergency items or services.
Make sure your health care provider gives you a Good Faith Estimate in writing at least one business day before your medical service or item. You can also ask your health care provider, and any other provider you choose for a Good Faith Estimate before you schedule an item or service.
If you receive a bill that is at least $400 more than your Good FaithEstimate, you can dispute the bill.
Make sure to save a copy or picture of your Good Faith Estimate.
